Job Summary
As a Multi Skilled Community Care Officer, you will play a vital role in supporting adults to regain and maintain their independence. You will conduct strengths-based assessments, reviews, and support planning, focusing on achieving meaningful outcomes. Role Responsibilities
In This Rewarding Role, You Will
Conduct assessments to identify individual strengths and needs. Develop and review support plans that promote independence. Identify reablement opportunities to help individuals regain skills. Provide short-term interventions that focus on achieving personal goals. Collaborate with other professionals and community services to ensure comprehensive support. Maintain accurate records and documentation in line with council policies.
Required Skills And Qualifications
To thrive in this role, you should have:
Have a relevant health or social care qualification (NVQ Level 3 or equivalent) or substantial relevant experience in adult social care, particularly in assessment and support planning. Knowledge of reablement principles and practices. A valid driving licence and access to a vehicle for work purposes.
