The Role

The post holder will support the District Nurse in delivering effective, holistic nursing care to a designated caseload within Inverclyde Health and Social Care Partnership (HSCP). The role contributes to the assessment, planning, implementation, and evaluation of care for individuals with a range of healthcare needs in the community setting.

The Community Nursing Service provides high-quality nursing care to the housebound population of Inverclyde. The service aims to deliver safe, effective, and person-centred care within patients’ own homes, supporting independence, dignity, and wellbeing in line with NHS Greater Glasgow & Clyde values.

Duration, Location, and Working Pattern:

Location based in Inverclyde HSCP Permanent Contract Full-time position (36 hours per week) Shift pattern for this post are 5 days over 7 including weekends, evenings and public holidays

Key Responsibilities

The post holder is responsible and accountable for assessing, planning and implementation of programmes of care as directed by district nurse. These will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health. The post holder may be required to assume the responsibility of the caseload holder during short periods of sickness, absence and annual leave, seeking support as required from other senior members of the district nursing The post holder will be responsible for the utilisation of various clinical and environmental risk assessment tools in order to identify actual and potential risks and ensure implementation of appropriate interventions. The post holder will be responsible for reporting and participating in the critical appraisal of incidents to reduce any recurrences and participate in further investigation as required. As a member of the district nursing team the post holder will be able to support patients, families and carers in order to achieve optimum health and independence. Plan and prioritise workload as required, delegate work using available resources and make decisions to ensure that patients’ needs are being met. Work within systems to ensure continuation of a quality 24-hour nursing service The post holder may be required to provide care of patients within a Treatment Room or Health Centre setting The post holder will be familiar with and ensure implementation of local, regional and national policies, procedures and guidelines pertinent to their area in the pursuit of the highest standard of care. Provide clinical leadership and be a role model for junior members of the team by representing the values and beliefs of the nursing profession as well as that of the organisation. The post holder will be responsible for the day-to-day management of their own workload as delegated by the district nursing sister/charge nurse. Knowledge, training, qualifications and/or experience required to do the job:

Essential

First-level Registered Nurse (Adult) with relevant post-registration experience Current registration with the Nursing and Midwifery Council (NMC) Ability to work independently and as part of a multidisciplinary team Excellent communication and interpersonal skills Full UK driving licence and access to a car

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
