This patient facing role is suitable for someone who enjoys educating and caring for others and is competent with communication technology. Ability to work independently is also important. The successful candidate will be required to adapt training and support according to patient age and ability. Some patients may require repeat appointments until they are comfortable and competent with the technology.
To work independently as part of the diabetes team, under the leadership, supervision and guidance of the Diabetes Specialist Nurses, in the support of people with diabetes in the hospital setting.
Manage a delegated caseload. provide education to patients (one-to-one and in groups) to promote self-management of diabetes &. To encourage and support people with diabetes to manage their condition more effectively with the use of glucose monitoring systems (worn technology) and smart pens. The role will include training on glucose monitoring, smart pens, mobile phone apps, email, enabling the sharing of data and supporting with any technical issues patients may have.
Responsible for service improvement initiatives. Collate and analyse the results from surveys and questionnaires to inform improvement work and present this in a range of formats for different staff groups/audiences.Support with ensuring data sharing prior to/at planned clinic appointments and teaching patients how to do this from home where possible
Support with technology groups and maintenance of spreadsheets/record keeping to include GP correspondence, communication with patients/family, company representatives, finance and payments teams.
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our Community Service for adults are provided in people's homes ,community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent , improve their health and manage their conditions through high quality care , advice and support.
