Since our inception in 2015 in Denmark, Liva has been a pioneer in the field of digital behaviour change. Our ambition is to empower 1 million people by 2030 to improve their health and well-being by preventing and managing chronic conditions through digitally-powered, evidence-based lifestyle interventions. These particular roles are required for Momenta Newcastle Limited, which is part of the Liva Group.

Momenta Newcastle is a leading UK provider of evidence-based health behaviour change programmes. Commissioned by NHS England, we deliver the 'Type 2 Diabetes Path to Remission Programme' across multiple regions. We're looking for a number of in-person Health & Wellbeing Coaches to work with us on a freelance project basis to help deliver our 1:1 NHS T2DR face-to-face sessions, supporting participants through a structured year-long journey toward Type 2 diabetes remission.

We are specifically looking for coaches to cover clinics in our North East England region. Clinics take place in and around Darlington and Middlesbrough - so you must be happy to commute to these locations to hold coaching sessions in person. What you’ll be doing

You will be supporting the delivery of the NHS Type 2 Diabetes Path to Remission programme through in-person, 1:1 coaching. You'll empower participants to improve their health outcomes over a structured 12-month journey, aiming for remission where possible. Delivering one-to-one sessions in local health or community venues in Darlington and Middlesbrough Building rapport and tailoring support to participant needs Supporting long-term behaviour change through structured coaching Documenting all sessions and tracking outcomes What You'll Have Experience with health coaching or health interventions A relevant qualification - either a degree in a health-related subject, or a relevant UK health coaching qualification Warm, empathetic and engaging communication style Ability to support a diverse range of patients through structured programme delivery Proficiency with handling patient data and using digital systems and technology Willingness to travel within your region (Darlington and Middlesbrough) to deliver clinics Able to join 3 days of training online prior to starting Registered as self-employed and with relevant insurance to deliver health coaching to UK-based clients

Desirable Experience delivering 1-1 health coaching Understanding of type 2 diabetes
