Description: Full Time / 37 Hours Per Week / Fixed Term for 2 Years We believe in supporting people and their families and carers to exercise choice and control in relation to how their needs are met. As a lead commissioner you'll manage strategic planning and commissioning projects that relate to a range of services which support adults aged 18 and over. Taking a lead on the development of our long-term strategic commissioning plans that enable us to deliver our vision for the people of Leicester. The Ageing Well lead commissioner post will manage the team who are responsible for our Ageing Well services, support and strategies in Leicester. This portfolio area leads on the commissioning of key services such as residential and nursing care for older people, and commissioned support services such as our carers support service and dementia support service which help people and their carers to live and age well in our city. The post takes also takes a lead on the commissioning of our discharge to assess services, ensuring we have an effective intermediate care offer for people being discharged from our hospitals. An important and essential part of the role, therefore, will be your ability to collaborate with key external partners, like our NHS and internal teams such as our occupational therapy and social work colleagues. Critical to the success of the work you do will be your ability to build effective partnerships with our voluntary sector and vibrant homecare and residential and nursing market. Underpinning all this will be your commitment to work in coproduction and codesign with people and carers, recognising people's strengths and ensuring their voices are at the heart of everything we do.
