Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. Compensation and Benefits: Earning potential: $110,000–$200,000 (salary based on experience + easily achieved monthly bonuses) Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays. Key Responsibilities: Assess, develop and communicate comprehensive treatment plans using evidence based best practices Documentation withing 24-48 hours along with placing relevant orders for diagnostics and medications Monitor and manage acute and chronic conditions Compliance with facility, state and federal regulations/protocols. Support quality initiatives and process improvement plans. Collaborate with interdisciplinary team, nurse managers, physicians and specialists Educate and consul patients and families regarding course of treatment QualificationsQualifications: Active license as a Nurse Practitioner (NP) or Physician Assistant (PA) Minimum 2 years of post-certification experience Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred Proficient with EHRs and clinical technology Strong communicator with a collaborative mindset #HP123
