Job Overview: The Nursing Associate will work as part of the multidisciplinary team (MDT) within Primary Care, delivering high-quality, compassionate, and person-centred care to patients. This role bridges the gap between Healthcare Support Workers and Registered Nurses, providing holistic care while ensuring patients are supported in managing their health and wellbeing. This post is funded through the Additional Roles Reimbursement Scheme (ARRS) and will support the delivery of the PCN’s objectives, enhancing capacity and improving patient access to care.

Role Specific Duties: Deliver a high quality, compassionate care to patients under the supervision of registered nurses and in accordance with the Nursing and Midwifery Code and Standards of Proficiency for Nursing Associates. Support patients throughout their outpatient journey, including diagnostics, minor procedures and follow up care. Monitor and record patient's vital signs, clinical observations and escalate concerns in line with clinical policies and procedures. Assist with minor clinical procedures and support nurses/GPs during diagnostic or interventional appointments. Perform venepuncture, ECGs, wound care, smear tests, adult and child immunisations and other clinical tasks within your scope of practice. Support in long-term condition reviews (as competency allows, e.g., asthma, COPD, diabetes support) Maintain accurate, timely, and complete patient documentation using the appropriate systems like EMIS, Docman and SystemOne . Contribute to quality improvement activities, audits, and service development projects within the practice. Compliance with infection prevention and control and health and safety regulations. Participate in clinical audits as required by the clinical staff. Ensure adequate stock levels to allow care to be provided.

Person Specifications: Essential NMC registration as a Nursing Associate Understanding of primary care, community care, or long-term condition management Experience with smear tests. Experience in carrying out adult and child immunisations. Trained on ILS Minimum 1 year experience as a Nursing associate working in a clinical environment. Strong communication and interpersonal skills Ability to work independently and as part of a team Commitment to continuous professional development Desirable Experience in a GP practice or community setting Knowledge of EMIS/TPP SystmOne or other clinical systems Experience delivering patient health education

They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. Under the Data Protection Act 1998, the Post Holder must maintain the confidentiality of information about patients and ensure confidential information is handled and kept safe and secure, following all information governance rules and regulations. Ensure compliance with legislation, policies and procedures both in the workplace and on home visits. Reporting any potential risks as they are identified.

Infection Control The post holder will undertake infection control training and update this periodically as required. The post holder will take part in all infection control measures within the practice following current procedures and protocols.
