Responsibilities

If you are a Physiotherapist with a passion for intermediate care based rehabilitation, with strong communication skills and the ability to affectively work as part of an MDT this may be the challenge you have been looking for. We have an exciting new opportunity for an enthusiastic, experienced and committed Physiotherapist with strong core physiotherapy skills and a desire to build on specialist skills in the delivery of rehabilitation within a multi-disciplinary team. The team comprises of nurses, occupational therapists, physiotherapists, assistant practitioners and rehabilitation assistants to join Locala’s Intermediate Care Bed Base team, with close links to acute wards as well as community colleagues. The service is based within a social care setting serving the residents of Kirklees. We offer a 7 day service, working 8am until 7pm providing assessment, treatment and evaluation of clients’ needs in a rehabilitation setting based on a patient centric programme. The ultimate aim being to prevent unnecessary future admission to hospital and in addition to ensure the swift appropriate discharge from hospital to offer rehabilitation and enable our clients to return home. The team will also work with extended reablement and reablement services in addition to the third sector, with the aim also of avoiding inappropriate admission to long term care. Working on initiative is essential, and there will be opportunities to support and skill share with colleagues within the team as well as across the different sectors. The role will incorporate consultations and education within the primary and secondary care environment, providing a specialist resource for health care professionals and patients, providing formal and informal training. The post holder will require excellent communication skills to build complex relationships and network with key stakeholders to enable integration. As a specialist physiotherapist you will contribute to service development and polices with the senior members of the team and give specialist advice to clients and other professionals as required, together with clinical supervision for junior colleagues and students. Whilst each professional will have their core skills, it is essential that all the team members work closely together in an integrated MDT to provide a comprehensive service through skill sharing, utilising the Calderdale Framework. We are committed to supporting CPD with staff training identified through our robust appraisal system. Supported by in service training, access to external courses and clinical supervision. INTERMEDIATE CARE BED BASED therapist will work closely with colleagues across Adult Community Therapy Services, START and Reablement to ensure clinical competencies are maintained and clinical pathways are robust to allow patient flow. This service is very much a team approach, with support in supervision, regular team meetings and training.
