We are delighted to offer this exciting opportunity to join the Community Directorate Pharmacy Team within our Wigtownshire Cluster.

Our team of pharmacists, pharmacy technicians and pharmacy support workers delivers the GP practice clinical pharmacy service across both GP practices and pharmacy hubs. The team plays a key role in medicines management and medication review as part of the primary care multidisciplinary team. This is a rewarding patient-facing role, supporting people across Dumfries and Galloway to use their medicines safely and effectively.

You will provide a clinical pharmacy service directly to patients. Day-to-day responsibilities may include:

  • Undertaking clinical assessment within GP practice clinics
  • Carrying out complex polypharmacy medication reviews
  • Assessing patients’ ability to manage their medicines, including within their own homes
  • Responding to medicines-related queries
  • Completing medicines reconciliation and prescribing medication

Collaborative working with GP practice staff, the wider Health and Social Care multidisciplinary team, acute services and community pharmacy colleagues is central to the role, ensuring the best possible outcomes for patients.
