An exciting opportunity has arisen for a Band 6 Specialist Physiotherapist to join the Rhins Home Team. This role offers the chance to work as part of a multi-disciplinary home team to support patients at home, prevent admission to an acute environment, and facilitate timely discharge from hospital.
- Can you inspire, enable, and empower individuals, Carers, and colleagues through compassionate conversations to work collaboratively to achieve what matters within improved health and wellbeing outcomes?
- Can you demonstrate your skills through re-treatment programs, supporting self-management, supporting mental well-being, and maximising independence?
- Can you confidently self-coordinate a complex caseload while proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners?
- Can you demonstrate effective working relationships across Locality Teams inclusive of GPs, Nursing, Social Work, Social Care, AHPs, Third and Independent sector organisations?
- Are you aware of how a Home Team works and demonstrate how you can contribute towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway?

- A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged.
- Due to the ever-changing health and social care climate, this post holder may be required in the future to work flexibly within the context of working regular weekends, to provide a 6 or 7 day per week service.
- There is also a requirement to regularly travel to different workplaces.
This is a fixed term post for 12 months. This will be discussed further at interview.
