Exciting Opportunity: Connect Edinburgh Programme

An exciting opportunity has become available to join the Connect Edinburgh programme. This programme aims to make it quicker and easier for people to get the support they need, when they need it, by improving how people access our services. This new way of working is currently in its design phase. As Specialist Practitioner District Nurse supporting this programme you will have a key role in developing approaches that help identify people at an earlier point in their hospital stay and streamline their discharge from hospital. By utilising an in-reach approach as SPDN you will actively seek patients that have the potential for discharge and will work with the MDTs to arrange the care and support needed for patients to safely return home. This will reduce occupancy and will transition from a traditional model of hospital prescribed care to care and support being arranged by the community teams that better know the services and community pathways available to them.
