A unique opportunity has arisen to join our award-winning team working across both hospital and primary care settings. We are looking for an enthusiastic pharmacist to contribute to our clinical pharmacy service delivery across Midlothian Community Hospital and Midlothian Primary Care.
About The Role
The role will be split across two exciting areas Midlothian Community Hospital providing patient-centred inpatient care and Midlothian Primary Care.

You will join a dedicated team delivering specialist clinical pharmacy services across Medicines of the Elderly rehabilitation, Hospital Based Complex Care and Psychiatry beds at Midlothian Community Hospital. The post holder will help develop and deliver the service, supporting challenges around capacity and patient flow, and will play a key role within a multidisciplinary team, including non-medical prescribing.
In addition, the successful candidate will work in general practices delivering services locally as well as centrally with our team in the medicine’s reconciliation hub. We require an experienced pharmacist to deliver a range of clinical pharmacy services including polypharmacy reviews. You will be responsible for monitoring service delivery and providing reports to key stakeholders including Health and Social Care Partnership clinical leads.

Requirements
Excellent leadership, communication and interpersonal skills are required. This post requires Enhanced PVG Scheme Membership.
