Job Opportunity: Patient Flow Coordinator
About the Role
An exciting opportunity has arisen to join the Patient Flow Team, based within Dumfries & Galloway Royal Infirmary and Regional Community Hospitals.
Working across acute and community services as part of a multidisciplinary team, you will play a pivotal role in coordinating patient journeys and supporting timely, safe discharges from hospital to home or other care settings across Dumfries and Galloway.

Responsibilities
This is a rewarding and challenging role where no two days are the same. You will work closely with a wide range of health and social care professionals to overcome barriers to discharge, coordinate complex care arrangements, and help ensure patients receive the right care, in the right place, at the right time.
Requirements
The successful candidate will have excellent communication, organisational, and problem-solving skills, with the ability to build effective relationships across services and work confidently in a dynamic environment.
