Community Navigator
The Community Navigator will support the administration provision for the District Nursing Team. They will be responsible for administrative duties of the District Nursing Team, ensuring these duties are maintained and delivered efficiently and effectively.
The Community Navigator will utilize a variety of IT functions and systems used in the District Nursing team, and have a wide knowledge of a range of clinical and non-clinical information systems.
The Community Navigator will have excellent communication skills, and communicate effectively with the District Nursing Team, the wider multi-disciplinary team, patients and their relatives, with the ability to signpost individuals to the appropriate service when necessary.
The post holder will be knowledgeable in Cwm Taf Morgannwg University Health Board (CTMUHB) policies and procedures.
Putting the patient at the forefront of every aspect of this role will be essential. The role will prove to be rewarding and the position will offer job satisfaction.
The community navigator will work closely with their district nursing team, the community navigator colleagues and the wider multi-disciplined team. Being part of the team is essential.

The community navigator is required to receive information, to relay information from and to a range of clinical systems. Therefore the need to be trained in the use of a range of clinical and non-clinical systems is key to the post along with the need to be an excellent communicator. The requirement is to provide effective written and verbal communication skills.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
