Leaders in Care is recruiting for a Social Worker to join a supportive, multidisciplinary Adult Front Door/Access service in the South East. This is an excellent opportunity to work with adults with complex needs while receiving consistent support throughout your placement.

You will work alongside professionals across social care, health, housing, and community services to help adults access the right support, retain their independence, and achieve positive outcomes.

What You'll Be Doing

  • Completing comprehensive, strengths-based assessments.
  • Creating person-centred support plans that reflect each individual's needs, goals, and circumstances.
  • Managing a varied caseload involving complex needs, safeguarding, and risk.
  • Supporting adults with mental health needs, learning disabilities, autism, and other complex circumstances.
  • Applying the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding legislation in practice.
  • Working closely with partner agencies to provide coordinated and effective support.
  • Promoting independence, choice, and wellbeing for the adults you support.

What We're Looking For

  • A recognised Social Work qualification.
  • Current Social Work England registration.
  • Previous experience in Adult Social Care.
  • Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding procedures.
