Job Title

Deliver personal care and support to elderly and vulnerable adults in their own homes across Hertfordshire, while working towards an Adult Care Worker L2 apprenticeship. You will travel between clients, work independently and provide reliable, compassionate and person-centred care.

Purpose of the Role

To provide safe, dignified and person-centred care to clients in their own homes, supporting their choices, independence and well-being.

Key Responsibilities

• Deliver care in line with each client’s care plan and risk assessment.
• Provide personal care, including bathing, dressing, grooming, oral care and continence support.
• Assist with medication and accurately complete MAR charts.
• Prepare food and drinks and support clients with nutrition and hydration.
• Support mobility, transfers and the safe use of equipment.
• Provide domestic assistance, companionship and support with social contact.
• Record all care visits accurately on Birdie at the time of the call.
• Report changes in a client’s health, needs or circumstances without delay.
• Follow safeguarding, infection control, moving and handling and lone-working procedures.
• Report emergencies, accidents, incidents, near misses and equipment faults promptly.
• Maintain confidentiality and communicate effectively with the office and wider team.
• Travel between clients’ homes and arrive on time for each scheduled visit.
• Wear the required uniform and ID badge.
• Complete mandatory training, competency checks, supervisions and appraisals.

• Use appropriate communication methods that are adapted to respond to the needs of individuals, stakeholders and family members in adult social care settings, including verbal, written, non-verbal and digital communication.
• Develop personal resilience and access support to maintain wellbeing of self.
• Use record keeping systems to maintain clear and accurate records of interactions with individuals, and others ensuring security of information and data.
• Apply methods of digital working and communication and new care technologies to support improvements in own work setting.
• Record learning opportunities in line with organisational policies that support lifelong learning and meet the relevant standards.
• Record, use feedback and supervision to improve own practice to identify and support the development of individual goals and career opportunities.
• Contribute and agree to the personal development plan demonstrating support required for new learning goals and reflection on actual practice.
