Adult Healthy Lifestyle Practitioner

At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier! It’s a super exciting time to join us to deliver our Integrated Health and Wellbeing Service (IHWS) throughout the City of Nottingham, and we’re looking for an Adult Healthy Lifestyle Practitioner to join the team!
Our IHWS programme is designed to support individuals and families to improve their health behaviours and make sustainable changes to their lifestyle. The programme consists of four key services:
- Stop smoking support
- Tailored physical activity programmes (this is where you come in!)
- Adult weight management (and here too!)
- Child/family weight management
Residents across the City of Nottingham will be able to benefit from a variety of our established pathways and products, including Gloji, Move More, Slimming World and MAN v FAT Football.

A typical day as an Adult Healthy Lifestyle Practitioner might include:
- Delivering evidence-based, group interventions covering nutrition, movement, mind, sleep and alcohol, with a focus on behaviour change and weight management.
- Developing and delivering a range of group interventions that focus on supporting clients to increase their physical activity levels.
- Motivating, inspiring, and supporting individuals to lose weight, be more active, and adopt a healthier lifestyle, through a professional, knowledgeable, and empathetic approach.
- Implementing aspects of an individual’s health improvement plan including SMART goal setting, taking into account their risk factors, individual lifestyles, and motivations.
- Ensuring an excellent and safe experience for clients, aligning with our internal policies and procedures.
- Supporting referral generation activities to promote the service including outreach and primary/secondary care liaison.
- Accurately recording and monitoring data, including client progression.
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements, and guidance.
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

To be considered for this role
You must hold a recognised Level 2 Gym Instructor qualification (or equivalent) and have experience supporting individuals in making healthy lifestyle choices, ideally in group settings, through nutrition, physical activity, or health coaching. You will also need to feel confident managing a caseload and have knowledge of behaviour change techniques.
Although not essential, so that we can best support the needs of our clients in Nottingham we are particularly keen to hear from applicants who are fluent in Polish, Urdu, Arabic, and/or Romanian.
However, we hire based on attitude over traditional skills and experience, so if you feel you are right for the job, we would love to hear from you!

Contract
We have 1 x Part-time (22.5 hours) available. This is a permanent position.
Salary
Starting at £14,871, rising through annual increments.

Location
This role is based in the City of Nottingham, with regular travel across the patch (car owner, preferable but not essential). The job will involve some home/office working when not delivering face-to-face sessions.
