We're recruiting for an Adult Nurse Case Manager to join a leading specialist complex care provider delivering high-quality care to adults with complex healthcare needs in their own homes across North West London.
This is an excellent opportunity to move away from shift work and into a Monday to Friday role offering autonomy, variety and the opportunity to build long-term relationships with service users while delivering outstanding clinical care.

The Role
As an Adult Nurse Case Manager, you will:
- Manage a caseload of adults with complex healthcare needs living within the community
- Complete comprehensive clinical assessments and regular care reviews
- Develop and review person-centred care plans
- Undertake risk assessments and clinical governance activities
- Complete competency assessments and clinical supervision for Healthcare Assistants
- Support the mobilisation of new care packages
- Liaise with families, GPs, Consultants, Commissioners and multidisciplinary teams
- Respond to clinical concerns and provide expert clinical guidance
- Support CQC compliance and maintain high clinical standards across your caseload
What We're Looking For
- Registered Adult Nurse (RGN/RN Adult) with a valid NMC PIN
- Experience supporting adults with complex healthcare needs
- Clinical skills in one or more of the following:
- Tracheostomy care
- Ventilation
- PEG feeding
- Spinal injuries
- Neurological conditions
- Complex respiratory care
- Experience within community nursing, complex care, case management, district nursing, ICU, HDU or acute hospital settings
- Excellent communication and clinical decision-making skills
- Full UK driving licence and access to a vehicle
