About the Role
Would you like to join a team providing positive lifestyle opportunities in a home-based community setting?
Sheffield Shared Lives is part of a UK wide network of schemes. We provide unique support for full-time placements, short breaks and day support.
To enable the team to support carers and people who use the service, develop, promote and raise awareness of this successful model we are seeking to recruit two Adult Social Care Practitioners.
You will have the opportunity to build strong relationships with our Shared Lives Carers and the people they support. Develop innovative support plans and work with a diverse range of people.
If you have experience of working with adults who have a wide range of care and support needs, supervision of staff/volunteers, excellent organisational/IT skills and a willingness to learn, we would like to hear from you.
You will be responsible for co-ordinating and managing a diverse caseload as well as supervising Shared Lives
You will work with the individuals they support, in partnership with their families, other professionals and key stakeholders. You will be an effective communicator who enjoys ‘thinking outside the box’, promoting a positive culture, developing innovative and high-quality services.

The Shared Lives service, as a model, keeps the person at the heart of the service and support they receive. The candidates will have a commitment to Liberty Safeguards and the principles of the Mental Capacity Act and Human Rights Act.
We offer flexible home working provision there will also be office-based function to participate in team meetings and regular home visiting to meet with those you support in the community.

Benefits
Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
