Our new Community Reablement Service is seeking committed, compassionate and motivated Social Care Practitioners to join our countywide team. This is a unique opportunity to be part of an innovative service from the very beginning, helping to build and influence how we deliver support to the people of Gloucestershire.

Working as part of an integrated multidisciplinary team, you will play a vital role in supporting people at a key point in their journey. When it seems an individual may require ongoing care and support beyond their reablement period, you will work alongside colleagues to undertake strengths-based Care Act assessments, identify eligible needs, and develop person-centred support plans that maximise independence and wellbeing.

About the Service

Our Community Reablement Service has been designed to help people regain confidence, skills and independence following a change in circumstances.

The service brings together professionals from a range of disciplines, working as one team to support people to achieve what matters most to them. We focus on enabling people to do as much as possible for themselves, promoting strengths, resilience and community connections.

As a Social Care Practitioner, you will be a key part of this integrated approach. You will work closely with Occupational Therapists, Reablement Assessors, Community Reablement Providers and wider Adult Social Care teams to ensure seamless transitions from reablement into longer-term support where required.

Most importantly, you will have the opportunity to influence and shape the future of this new service, helping us develop innovative approaches that deliver the best outcomes for the people we support.

About the Role

You will:

  • Undertake strengths-based Care Act assessments with adults who may require longer-term care and support.
  • Work alongside individuals, carers and families to identify outcomes, maximise independence and develop effective support plans.
  • Support people to access community resources, informal networks and local services that promote wellbeing and independence.
  • Work collaboratively within an integrated multidisciplinary team, ensuring a coordinated approach to support planning.
  • Apply the principles of the Care Act, Mental Capacity Act and safeguarding legislation within day-to-day practice.
  • Support individuals through periods of change, helping them make informed decisions about their future care and support needs.
  • Contribute to the ongoing development of this new service through innovation, reflection and continuous improvement.
  • Be an advocate for strengths-based practice and person-centred approaches.

The role will be based at Shire Hall, Gloucester, with travel across Gloucestershire as part of our countywide service delivery model.

About You

We are looking for people who:

  • Have experience working within health, social care or a related setting.
  • Are passionate about delivering person-centred, strengths-based support.
  • Have experience of carrying out assessments and developing support plans or can demonstrate the skills and potential to do so.
  • Are confident working with individuals, families and partner organisations.
  • Can build effective relationships and navigate challenging conversations with empathy and professionalism.
  • Understand, or are willing to develop their knowledge of, the Care Act, Mental Capacity Act and safeguarding responsibilities.
  • Are organised, resilient and able to manage a varied workload.
  • Enjoy working collaboratively and contributing ideas to service development.
  • Want to be part of an ambitious and evolving service that is focused on achieving excellent outcomes.
