Adult Social Care Practitioner
Salary: Grade 12, £8,783 to £9,847 per annum, pro-rata (Full time Equivalent £29,542 to £33,119 per annum).
Contract: Permanent
Hours: Part-time, 11 hours per week
Location: Taunton

About the Role
We are looking for a compassionate and motivated Adult Social Care Practitioner to join our team in Taunton on a part-time basis, working 11 hours per week.
You will work alongside adults, carers, colleagues and partner organisations to assess needs, review care and support arrangements, and identify strengths-based solutions that promote independence. Working under the supervision of a manager, you will help ensure that proportionate and sustainable support is in place in line with the Care Act 2014.
How You'll Make a Difference
- Complete assessments and reviews with adults and carers, focusing on their strengths, goals and the resources available within their communities.
- Provide clear information and advice about community services, health, housing, welfare benefits and voluntary or user-led support.
- Co-produce personalised, outcome-focused support plans, including direct payments and commissioned services where appropriate.
- Manage an agreed caseload, organise and prioritise your work, and respond appropriately to changing levels of risk and need.
- Work closely with Social Workers, Occupational Therapists, health colleagues, providers and other partners to coordinate effective support.
- Recognise and escalate safeguarding, capacity or high-risk concerns, seeking professional guidance when required.
- Maintain accurate and timely records and contribute to peer forums, audits, supervision and ongoing service improvement.

What You'll Bring
- Experience of working in a caring or supportive role, whether paid or voluntary.
- An understanding of the rights of adults and carers, with the confidence to advocate on their behalf.
- Empathy, respect and the ability to adapt your communication to meet individual needs.
- The ability to work independently, use your initiative and balance competing priorities.
- Good literacy, numeracy and IT skills, including the ability to maintain accurate records.
- A commitment to learning, reflective practice, supervision and your own professional development.
- The ability to travel to the homes of adults and carers as required for the role.

Further Information
This is a part-time role offering 11 hours per week. The working pattern will be agreed with the successful candidate in line with service needs.

The role is based in Taunton and will involve travel to meet adults and carers in their homes and community settings.
Due to the nature of the role and the geographical area of the service across Somerset, which is a largely rural county, with limitations around public transport the ability to be able to travel to any location is essential. Therefore, applicants must have full access to a vehicle that can be used for work purposes.
