Xander Recruitment Group LTD is an approved NHS Framework Supplier. We are currently recruiting for an advanced clinical pharmacist (Acute medicine and admissions) on behalf of our client, an established NHS Trust on the Isle of Wight.
The Role
The post holder will lead, develop, deliver and evaluate a consistently high-quality clinical pharmacy and medicines optimisation service to patients within acute medicine, focusing on newly admitted patients across the Emergency Floor which includes the Acute Admissions Unit (AAU), Same Day Emergency Care (SDEC) service and Emergency Department (ED). They will provide expert pharmaceutical advice for the management of patients, deliver high quality pharmaceutical care, improve patient safety and patient flow. They will ensure quality assurance, clinical governance and risk management of medicines in support of Trust targets and national requirements. To ensure safe prescribing, the post holder will be expected to produce and maintain policies and guidelines.

The post-holder will be an independent prescriber with health assessment skills (or working towards these qualifications), responsible for undertaking medicines reconciliation and medicines optimisation by utilising their prescribing qualification and expert pharmaceutical knowledge to review and prescribe patients’ regular medication. They will also independently prescribe medication in order to facilitate discharges directly from the Emergency Floor.
They will undertake clinical supervision and training of rotational clinical pharmacists, trainee pharmacists, medicines optimisation technicians, student technicians and provide education and training on medication to colleagues within the multi-disciplinary team.

About You
- Master’s Degree in Pharmacy or equivalent
- Registration with the General Pharmaceutical Council (GPhC)
- Diploma in Clinical Pharmacy Practice or equivalent experience
- Completed General Level Foundation Framework competency training
- Significant experience of working in a pharmaceutical environment
- Significant clinical pharmacy knowledge and experience
- Significant experience of working in hospital pharmacy
- Knowledge of national guidelines relating to medicines and pharmaceutical practice
- Specialist knowledge and experience within a relevant clinical speciality
- Ability to lead change and hold others to account
- Demonstrate an understanding of commissioning dynamics
