This post is based in the pharmacy, children’s out-patient clinics and on the specialist liver/renal ward at Leeds General Infirmary, although you may be required to work at other sites to support the service when necessary.

The main duties for staff working in this job, alongside those outlined in the job description, are:

  • To provide a highly specialised clinical pharmacy service to paediatric liver patients at ward level, and both liver and renal transplant patients in outpatient clinics as part of a team including a paediatric consultant pharmacist, other advanced clinical pharmacists (ACPs) and a Specialist Clinical Pharmacy Technician. This will involve close working with the MDT on ward rounds and in clinic, to support medicines optimisation, patient centred care and management of the homecare service to ensure safe and timely supply of immunosuppression.
  • To collaborate on the development of homecare provision of immunosuppression for all paediatric solid organ transplant patients across the region.

This is a new position to work collaboratively alongside the Paediatric Consultant Pharmacist and other ACPs to develop Paediatric Liver and Renal Services including setting up an immunosuppression homecare service for all paediatric solid-organ transplant recipients across the regional/supra-regional areas and ensure safe, timely and efficient provision of medicines. A brand switching project will also be undertaken once all patients medicines supply has been repatriated to LTHT. The role is highly clinical with close MDT working both on ward rounds and in clinics.
