An exciting opportunity has arisen for an experienced and motivated ACP to join our growing Virtual Ward Service.

The Virtual Ward operates virtual beds across Nottinghamshire, delivering hospital-level care in patients' own homes through both admission avoidance and early supported discharge pathways. Working in partnership with Nottingham University Hospitals NHS Trust, Sherwood Forest Hospitals NHS Foundation Trust, Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust, NEMS and primary care colleagues, the service plays a vital role in supporting system flow and reducing unnecessary hospital admissions.

As an ACP, you will work autonomously as an expert clinician, managing a complex caseload of patients who would otherwise require acute hospital admission. You will provide advanced assessment, diagnosis, treatment and clinical decision-making whilst working as part of a highly skilled multidisciplinary team.

This role offers a unique opportunity to influence the future development of Virtual Wards across Nottinghamshire, contributing to service improvement, education, leadership and research whilst delivering outstanding patient care.

Responsibilities

• Independently assess, diagnose, treat and manage patients with acute and complex healthcare needs within your scope of practice.
• Undertake advanced clinical assessment and decision-making for patients admitted to Virtual Ward pathways.
• Prescribe, review and optimise treatment plans in accordance with non-medical prescribing qualifications and professional standards.
• Manage patients across both admission avoidance and early supported discharge pathways.
• Use remote monitoring technology, clinical assessment and diagnostic reasoning to identify deterioration and initiate appropriate interventions.
• Participate in consultant-led multidisciplinary team discussions and support clinical decision making.
• Provide clinical leadership, advice and support to members of the wider multidisciplinary team.
• Support the development of evidence-based pathways, policies and clinical standards.
• Lead and contribute to quality improvement, audit, research and service evaluation activities.
• Support education, supervision and development of colleagues across the Virtual Ward workforce.
• Develop strong working relationships with acute, community, primary care and ambulance service partners to ensure integrated patient care.
