Are you an experienced Advanced Clinical Practitioner looking for your next challenge in urgent and crisis mental health care?
We are seeking two dynamic, innovative and highly skilled ACPs to join South West London and St George's Mental Health NHS Trust at a pivotal time of transformation. We have opportunities within both our established Richmond Home Treatment Team (HTT) and our brand-new Crisis Access Hub, enabling successful candidates to play a leading role in shaping the future of mental health crisis care across South West London.
As an ACP, you will work autonomously within multidisciplinary teams, applying advanced clinical expertise across assessment, formulation, diagnosis, treatment, prescribing, leadership, education and quality improvement. You will provide expert clinical decision-making for individuals experiencing acute mental health crises, support the development of colleagues and contribute to service improvement and innovation.
Two Distinct Opportunities
Richmond Home Treatment Team
Join an established and highly regarded Home Treatment Team providing intensive community-based support as an alternative to hospital admission. You will undertake advanced mental health assessments, lead treatment planning, provide non-medical prescribing, support safe discharge planning and act as a senior clinical leader within the team.

NEW Crisis Access Hub
This is a rare opportunity to join a flagship service from the beginning. The Crisis Access Hub will provide a single point of access for mental health crisis referrals across Kingston, Merton, Richmond, Sutton and Wandsworth. Working alongside senior clinicians and system partners, you will undertake advanced clinical screening, enhanced triage, risk assessment and pathway decision-making to ensure patients receive timely access to the most appropriate service.
You will help shape a new model of crisis care, working closely with NHS 111, ambulance services, acute hospitals, community mental health teams, Home Treatment Teams, primary care, social care and voluntary sector partners.
