Exciting Opportunity: Advanced Clinical Practitioners – Integrated Specialist Palliative Care Team (2 Posts)

Due to service expansion, we are seeking two motivated and compassionate Advanced Clinical Practitioners (ACPs) to join our Integrated Specialist Palliative Care Team.

This is an exciting opportunity to work across inpatient, outpatient, community, and virtual ward settings, providing expert assessment, symptom management, and care for patients with complex palliative and end-of-life care needs. The role includes supporting home visits as part of our innovative Palliative Care Virtual Ward, helping patients receive high-quality care in their preferred place of care.

As autonomous practitioners, ACPs work collaboratively with consultants and the wider multidisciplinary team, acting as clinical leaders and expert resources. We are looking for clinicians with strong advanced assessment and decision-making skills, excellent communication abilities, and a passion for delivering person-centred care.

The role also offers opportunities to contribute to service development, quality improvement, audit, research, education, and training.

Key Responsibilities

  • Assess patients and plan, implement, and evaluate care using specialist clinical knowledge and skills
  • Review patients and provide care in inpatient, outpatient, Virtual Ward setting, and home visits
  • Contribute to multidisciplinary approach to patient management
  • Provide education and support to patients, carers, and colleagues
  • Participate in audit, research, and service development within the Specialist Palliative Care service

Broadgreen Hospital is home to elective surgical, diagnostic, and treatment services, together with specialist patient rehabilitation. Liverpool Women’s Hospital specialises in the health of women and babies, delivering over 7,200 babies in the UK’s largest single site maternity hospital each year. The Royal Liverpool University Hospital is the largest hospital in the country to provide inpatients with 100% single bedrooms and focuses on complex planned care and specialist services.
