This is an exciting opportunity to work in our Urgent Care Home Visiting Team.
Successful candidates will need to demonstrate that they are able to meet the essential criteria in the person specification and can provide evidence of significant experience in their specialist field. You will be required to have an MSc in Advanced Clinical Practice or have completed the Health Education England supported portfolio route to accreditation. Candidates will also hold a non-medical prescribing qualification where appropriate.
We are looking for people who can demonstrate the right values and behaviours and have excellent leadership and communications skills. Portfolio evidence of core capability across all four Pillars of Advanced Practice is expected.
As part of a team, you will work as an autonomous practitioner, being accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. The role will include:
- Developing audit and research projects
- Providing ongoing supervision and support to medical and non-medical colleagues
- Playing a pivotal role in the operational development of their specialty

Role Details
This post is within our Urgent Care Home Visiting Team which supports patients that require an urgent healthcare need. The service is 24/7, with ACP shift patterns of 0800-2030, 7 days a week.
