As an Advanced Nurse Specialist - Continuing Healthcare, you will play a key role in leading complex assessments, coordinating care for individuals with highly complex needs and supporting a dedicated team of CHC nurses across Wiltshire. This is a rare opportunity to join a respected specialist service where your expertise will influence care decisions, shape service delivery and improve outcomes for adults with learning disabilities.

Base: Flexible across Marlborough, Chippenham, Warminster or Salisbury
Hours: 37.5 hours per week
Working Pattern: Hybrid working model (approximately 60/40), dependent on individual and service needs.

Lead, Influence and Improve Lives Across Wiltshire
Our Learning Disabilities Service is dedicated to promoting equal access to mainstream health services while providing specialist assessment, intervention and consultation for people with learning disabilities. Working as part of an integrated multidisciplinary team, we collaborate closely with Wiltshire Council, primary care services, specialist providers and the Integrated Care Board to deliver outstanding care and improve outcomes for individuals with complex needs.
How You'll Lead And Make a Difference
- Lead and support a dedicated team of Continuing Healthcare Nurses, providing clinical guidance, supervision and professional support.
- Coordinate and oversee complex CHC assessments, reviews and eligibility processes to ensure people receive the right care and support.
- Manage complex caseloads, helping individuals with learning disabilities achieve the best possible health and care outcomes.
- Build strong relationships with families, carers and partner organisations to deliver person-centred care.
- Support and develop team members, sharing expertise and encouraging professional growth.
- Participate in multidisciplinary meetings and safeguarding processes to advocate for individuals with complex needs.
- Help ensure care packages remain effective, safe and responsive to changing needs.
- Contribute to service improvement initiatives that enhance outcomes and experiences for service users.
What You'll Bring to the Team
We're looking for a compassionate and experienced nurse who is passionate about improving the lives of people with learning disabilities. You'll be a confident leader, skilled clinician and collaborative team player who enjoys supporting others and driving high-quality, person-centred care.
Essential Criteria
- Current NMC registration as a Registered Nurse.
- Significant post-registration experience working with people with learning disabilities and complex needs.
- Extensive knowledge and practical experience of the NHS Continuing Healthcare (CHC) National Framework.
- Experience completing CHC Checklists, Decision Support Tools (DSTs), reviews and complex eligibility assessments.
- Proven experience managing complex cases and coordinating large care packages.
- Experience leading, supervising and supporting staff.
- Strong clinical assessment, risk management and decision-making skills.
- Excellent communication, influencing and partnership-working abilities.
- Experience working within multidisciplinary and multi-agency teams.
- Knowledge of safeguarding, the Mental Capacity Act, Best Interests processes and positive behavioural support.
- Ability to manage a varied and demanding workload effectively.

Desirable Criteria
- Leadership or management qualification.
- Experience contributing to service development or quality improvement initiatives.
- Experience working with Integrated Care Boards (ICBs) and local authority partners.
- Knowledge of commissioning processes and care package monitoring.
- Experience chairing complex case meetings or safeguarding discussions.
