Job Overview
We have an exciting opportunity to join the VFC APP triage team, within the MSK South Physiotherapy Team at FHFT. The role involves daily close MDT working with the orthopaedic consultant team and the wider fracture clinic MDT, to manage all FPH fracture patients within the Virtual Fracture Clinic service in line with BOAST guidelines of fracture management.
As an Advanced Physiotherapy Practitioner, you will be expected to work across the four pillars: Clinical Practice, Leadership and Management, Education, and Research.
Main duties of the job
Role
The primary role of an Advanced Physiotherapy Practitioner - VFC Triage, is to review all notes and X-rays of patients on the VFC triage list, that have presented to ED or urgent care over the previous 24-48 hours, to:
- Ensure that correct management has been completed
- Determine whether the patient needs review in face to face fracture clinic; can be managed virtually with a telephone call and advice; can be managed with virtual call and physiotherapy treatment; or need to have an operation and be brought in urgently
- Order any additional imaging necessary to determine correct treatment pathway as above
They will work within their scope of practice, with daily contact with the on-call consultant, to jointly review any complex cases and those needing surgery.
They will oversee management of the VFC service to include communication with orthopaedic consultants, ED, radiology, the VFC physiotherapy and VFC admin teams. They will also be involved in conversations across site with the Wexham VFC team and with T&O management, regarding the structure and running of the service and necessary future developments.
They will be expected to lead on data collection and audit relating to VFC referral rates and service re-design and collectively manage annual leave cover of VFC triage, amongst the VFC APP triage team.

Detailed Job Description And Main Responsibilities
- Establish/develop cross-site specialist clinical services, engaging and influencing a range of key stakeholders across multiple professions and leading a small team of therapists within specialist area.
- Provide an independent expert assessment, diagnosis, treatment, prognosis and advice to a highly complex and diverse range of patients.
- Act as a source of clinical expertise and highly advanced knowledge for physiotherapists and other health professionals.
- In conjunction with other members of the MDT, to lead in various components of the development of the appropriate services and play an innovative and active part in developing the clinical service.
- To co-ordinate research and development activity as a major job requirement, providing information into local and national frameworks/documents.
- To provide expert advice and guidance through education both within the organisation and externally.

Qualifications
Essential criteria
- BSc or equivalent in Physiotherapy
- MSc module in Fundamentals of Musculoskeletal Radiographic Image Interpretation, or equivalent
- HCPC membership
Desirable criteria
- FHFT IMER training completed
- Membership of relevant clinical interest group
Experience
Essential criteria
- Experience working at Band 7 level in a VFC service
- Experience of working closely with T&O consultants
- Experience of close and effective MDT working
- Audit experience
Desirable criteria
- Experience or service development project
Skills and Knowledge
