Advanced Social Work Practitioner

Liquid Personnel is recruiting an Advanced Social Work Practitioner for its client at Nottinghamshire.

This is an excellent opportunity for a highly skilled practitioner to work within a fast-paced hospital setting, supporting adults with complex care and support needs across urgent and emergency care pathways.

The successful candidate will play a key leadership role within the team, supporting safe and timely hospital discharges, reducing delays, and promoting positive outcomes for individuals and their careers. Working closely with health professionals, multidisciplinary teams, community services, and partner agencies, you will provide expert social work practice, support complex decision-making, and contribute to the ongoing development of high-quality strengths-based practice across the service.

This role requires an experienced practitioner with a strong understanding of hospital discharge pathways, the Care Act 2014, Mental Capacity Act 2005, and Discharge to Assess guidance. You will also support the operational management of the team through supervision, coaching, performance management, and deputising for the Team Manager when required.

Responsibilities

• Managing complex cases involving adults with care and support needs within urgent and emergency care settings.
• Undertaking Care Act 2014 assessments, reviews, support planning, and risk assessments to facilitate safe and effective discharge planning.
• Applying the Mental Capacity Act 2005, including complex Mental Capacity Assessments and Best Interest decision-making.
• Supporting timely hospital discharge through effective application of Discharge to Assess principles and pathways.
• Working collaboratively with hospital clinicians, therapists, community services, care providers, carers, and partner agencies to coordinate support arrangements.
• Promoting strengths-based practice that maximises independence, wellbeing, and positive outcomes for individuals and their families.
• Assessing, managing, and mitigating risks whilst ensuring defensible and proportionate decision-making.
• Supporting the reduction of delayed transfers of care by proactively identifying and resolving barriers to discharge.
• Providing expert social work advice, guidance, and professional leadership within the multidisciplinary team.
• Supervising, coaching, mentoring, and supporting Social Workers and other Adult Social Care practitioners to improve practice and performance.
• Supporting the management team in driving service improvements, quality assurance activity, and professional development.
• Monitoring and supporting performance management processes, promoting accountability and best practice across the team.
• Chairing multidisciplinary meetings, discharge planning meetings, safeguarding discussions, and professional forums.
• Deputising for the Team Manager as required and contributing to operational leadership within the service.
• Maintaining accurate, timely, and legally compliant case records, assessments, reports, and professional documentation.
