Role Description
We have two posts available - one in Bedford and one in Luton.
The Agency Social Worker (Band 6) will work as part of a multidisciplinary team in Luton Older Adults CMHT or Bedford Older Adults CMHT providing high-quality social work support to service users and their families in Luton. This full-time, on-site role involves:

- Undertaking comprehensive assessments
- Developing person-centred care and support plans
- Coordinating interventions that promote safety, recovery, and wellbeing
The postholder will:
- Manage a caseload
- Complete risk assessments
- Contribute to discharge planning
- Ensure appropriate safeguarding measures are in place
Daily tasks include:
- Liaising with internal clinical teams, community services, local authorities, and voluntary agencies to ensure continuity of care and timely access to support
- Maintaining accurate records
- Participating in case reviews and multidisciplinary meetings
- Adhering to professional standards, Trust policies, and legal frameworks
The social worker will actively promote the Trust’s values and support service users to exercise choice, independence, and involvement in their care.

Qualifications
- Possess a recognized social work qualification (e.g., BA or MA in Social Work) and registration with Social Work England
- Possess skills in comprehensive assessment, care planning, risk management, and safeguarding adults
- Possess skills in mental health and community social work practice, including working with complex needs and multi-agency coordination
- Possess strong communication, advocacy, and interpersonal skills to engage service users, families, and partner organizations
- Possess skills in record keeping, report writing, and the use of electronic health record systems in line with confidentiality and data protection requirements
- Possess the ability to work collaboratively in multidisciplinary teams, manage a varied caseload, and prioritize workload effectively
Experience working within NHS or similar health and social care settings, and familiarity with relevant legislation (e.g., Mental Health Act, Care Act) would be beneficial.
