Foundation Apprenticeships
Foundation apprenticeships are introductory courses that help young people get started in an industry. You do not need to have any specific qualifications or experience to apply. Anyone between 16 and 21 can start a foundation apprenticeship. If you're between 22 and 24, you can start if you:
- Have an EHC plan
- Are in care or have been in care
- Are in prison or have been in prison
Level 2 Health and Social Care
Level 2 Health and Social Care is an entry-level, UK-recognised vocational qualification (equivalent to GCSE level) that teaches the basic skills needed to work as a care assistant, support worker, or healthcare assistant.
Wage
- £15,392 for your first year, then could increase depending on your age
- National Minimum Wage rate for apprentices

Training Course
- Health and social care foundation apprenticeship (level 2)
Hours
- 9.00am - 5.00pm/10.00am - 6.00pm/10.00am - 2.00pm/2.00pm - 6.00pm, Monday-Sunday, on a rotating shift pattern.
- 37 hours a week

Most of your apprenticeship is spent working. You’ll learn on the job by getting hands-on experience.
Job Purpose
Under the general direction of a Manager or Delegated Member of Staff, supports each individual Service User assigned to them to live safely and independently in the care home, and to achieve the goals and objectives of the person-centred care plan which has been produced and agreed with them. Each Plan is geared to maximising the lifestyle choices available to the Service User, and to achieving measurable improvements in their status, dignity, competence, and independence.
Key Tasks
Care Planning
- Assists with and participates in discussions with Service Users, or potential Service Users, in order to establish the key areas where support/care would be beneficial.
- Assists in the identification of personal, positive, and beneficial support/care to Service Users so that their goals, aspirations, objectives, and lifestyle choices can be identified, evaluated, and incorporated into their care plan, where it is realistic to do so.
- Working with the Manager or Delegated Member of Staff, and/or the Service Users, identifies, discusses, agrees, and records the actions which are (or may be) necessary to be taken in order that the care plan has a reasonable prospect of success.
- Assists in the preparation of care plans, and in final discussions with the Service User.

Implementation of Care Plans
- Participates in discussions with the Service User targeted at ensuring that the match between the Service User and the Support Worker is appropriate to the Service User’s needs.
- For assigned Service Users, reviews their Care Plan, and raises any queries with the manager or Delegated Member of Staff and/or the Service User as appropriate.
- Implements the Care Plan. Individual support needs will vary between Service Users, but support will always be given with the consent of the Service User, and with the aim of facilitating independence at the earliest possible opportunity and with the Service User leading the process, as much as possible. A list of sample Support tasks is attached.
Care Plan Review
- In conjunction with the Manager or Delegated Member of Staff, reviews the Care Plan with the Service User on a regular basis.
- With the agreement of the Service User, makes changes, modifications, or additions to the Plan so as to manage ineffective or changing situations.
- Implements the agreed changes.
Administration and Miscellaneous
- Maintains accurate and up-to-date records as required in relation to the services provided to the Service User, including tasks undertaken, training given, etc.
- Attends and participates in staff meetings and team briefings so that team members may benefit from the experiences of others, and contribute to service development and improvement.
- Reports accidents, near misses, unusual incidents, etc., in line with agreed and laid-down procedures.
- Participates actively and positively in personal appraisal, performance, and review processes.
- Attends training activities as arranged.

Training Schedule
The apprentice will attend Waltham Forest College, located at 707 Forest Road, Walthamstow, London, E17 4JB, as part of their apprenticeship training.
- 1 day per week at college
- On-site college-based training
The remaining days will be spent working with Connifinn Care, gaining practical experience, and developing social care and mental health skills in the workplace. Training will support the apprentice in building the knowledge, skills, and behaviors required to successfully complete their apprenticeship.
