Assistant Practitioner
Role Overview
11259 | Fixed Term Contract for 12 months | 37 hours per week | £33,119 to £34,811 per annum | Grade G | Hellesdon Hospital, Norwich
An exciting opportunity has arisen to join the Adult Mental Health Social Care Duty team. You will play a key role in delivering Community-Led Support (CLS) within Adult Mental Health Social Care. You will work alongside individuals, families, communities, and partner agencies to promote independence, recovery, wellbeing, and social inclusion through relationship-based practice.
You will focus on preventing, reducing, and delaying the need for formal care and support by working creatively with people, recognising strengths, building community connections, and supporting access to local assets and opportunities. You will develop meaningful relationships with individuals experiencing mental health challenges and support them to achieve outcomes that matter to them.
Working within a recovery-focused framework, you will help people identify and build on their existing strengths, networks, and resources while promoting autonomy, positive risk-taking, and social recovery.
Whilst the role forms part of a countywide Adult Mental Health Service, the primary focus of this post will be within the Norwich locality, reflecting the increased demand for Community-Led Support and relational practice interventions within the city area.
Key Responsibilities
Community-Led Support and Relational Practice
- Build trusting and effective relationships with individuals experiencing mental health difficulties, recognising that sustainable change happens through strong relationships and community connections.
- Engage with people in their communities, homes, and local settings, taking a proactive and flexible approach to support.
- Work alongside individuals to understand "what matters" to them rather than focusing solely on presenting problems or service-led solutions.
- Promote social recovery through strengthening connections with family, friends, peer networks, and community resources.
- Support individuals to access community groups, activities, volunteering, education, employment, and other opportunities that enhance wellbeing and social inclusion.
- Utilise asset-based approaches that recognise people's strengths, abilities, and aspirations.
