This is an exciting opportunity to join our Continuing Healthcare (CHC) service as an Assistant Team Manager, helping to shape excellent social work practice while supporting adults with significant and often complex health needs.
Working alongside the Team Manager, you'll play a key role in creating a positive team culture where people feel supported, valued and empowered to do their best work. The team works closely with individuals, families, carers, NHS colleagues and Integrated Care Boards (ICBs) to help people navigate the Continuing Healthcare process, ensuring their needs, wishes and outcomes remain at the heart of decision-making. NHS Continuing Healthcare is a package of care arranged and fully funded by the NHS for adults who have been assessed as having a primary health need.
This role is ideal for someone who enjoys developing others, building confidence and creating the conditions for great social work to thrive. Alongside supporting practitioners with complex work, you'll have the opportunity to influence practice, drive continuous improvement and help shape a service that makes a meaningful difference to the lives of local people.
Whether you're an experienced Assistant Team Manager looking for a fresh challenge or a Social Worker ready to take the next step into leadership, you'll join a collaborative and supportive service that values learning, reflection and partnership working across health and social care.
This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate. It is also a regulated activity and will be subject to a Children and Adults Barred List check. This is a varied role where supporting people and supporting practice go hand in hand.
Day to day, you'll provide guidance and oversight to social workers and practitioners, helping them deliver high-quality support to adults with complex health and social care needs. You'll work closely with the Team Manager to promote good practice, support professional development and ensure positive outcomes for the people we work with.
The Role Will Include
- Providing supervision, coaching and professional support to team members.
- Offering advice and guidance on complex cases, safeguarding concerns and risk management.
- Supporting the team to deliver strengths-based and person-centred practice.
- Working closely with NHS colleagues and other partners involved in the Continuing Healthcare process.
- Helping to manage workloads and ensure people receive timely support.
- Leading on quality assurance activity, audits and practice development.
- Supporting recruitment, induction and the development of colleagues.
- Contributing to service improvements and helping shape the future of the CHC service.

As a qualified Social Worker registered with Social Work England, you'll have a strong understanding of adult social care and a passion for delivering person-centred support. Above all, you'll share our commitment to treating people with dignity, respect and kindness.
