Role Description
Associate and/or Contracted Case Managers will work full time on-site in the United Kingdom, supporting adults following serious accidents and traumatic injuries. Day-to-day responsibilities include:
- Conducting comprehensive assessments
- Developing individualized rehabilitation and care plans
- Coordinating medical, therapeutic, and social support services

The role involves:

- Liaising with clients, families, healthcare professionals, insurers, and other stakeholders to ensure seamless delivery of treatment and services
- Monitoring progress
- Reviewing and adjusting plans as needed
- Providing advocacy for clients
- Ensuring that interventions promote independence, dignity, and informed decision-making
Documentation, report writing, and compliance with regulatory and company standards are also integral parts of the role.
Qualifications
- 5 years professional background in health or social care (e.g., nursing, occupational therapy, physiotherapy, social work) with experience in rehabilitation or case management.
- Strong assessment, care planning, and coordination skills, including the ability to manage complex cases and multidisciplinary input.
- Excellent communication, interpersonal, and advocacy skills to work effectively with clients, families, and a range of professionals.
- Solid organizational and time-management abilities, including caseload management, accurate documentation, and report writing.
- Knowledge of UK healthcare, social care, and community services, including relevant legislation, safeguarding practices, and clinical governance.
- Comfort working on-site in varied environments, with a person-centered approach that respects diversity, autonomy, and individual goals.
- Proficiency with basic IT tools and case management systems for maintaining records and communicating with stakeholders.
- Relevant degree or equivalent qualification in a health or social care discipline; registration with an appropriate professional body is advantageous.
- Experience working with people who have sustained traumatic injuries or long-term conditions is highly beneficial.
