Rehabilitation for Independence is an independent Case Management and Occupational Therapy company supporting children and adults with a wide range of neurological and physical difficulties across the North of England. The highly specialized therapy team delivers client-directed, holistic services in homes, workplaces, nurseries, schools, colleges, and community settings to maximize independence. The organization focuses on each client’s development, social situation, and family circumstances, providing individualized support tailored to specific needs and goals. Support workers are skilled communicators who build strong rapport with clients, families, and professionals, helping clients make the most of their rehabilitation journey and improve quality of life. The team collaborates closely with clinicians, medics, and other professionals to coordinate rehabilitation goals and deliver integrated, client-centered care.
Role Description
The Associate Case Manager is a full-time, on-site role based in the Manchester Area, United Kingdom. In this position, the individual will support the coordination and delivery of rehabilitation services, working alongside occupational therapists, support workers, and other professionals to implement client-centered care plans.

- Gather and organize client information
- Support assessments
- Monitor progress and document outcomes in line with clinical and regulatory standards
- Help arrange and attend multi-disciplinary meetings
- Communicate with clients and families
- Liaise with external agencies to ensure continuity of care
- Assist with goal setting, scheduling services, and responding to changing client needs
- Contribute to quality improvement initiatives within the service
Qualifications
- Demonstrated skills in client support and case coordination, including building rapport, maintaining professional boundaries, and supporting individuals with complex needs.
- Strong communication abilities, including clear written documentation, confident verbal communication, and effective collaboration with families and multi-disciplinary teams.
- Organizational and administrative skills, such as maintaining accurate records, managing schedules, and prioritizing tasks in a busy clinical environment.
- Understanding of rehabilitation, occupational therapy, or health and social care practices, with the ability to work within individualized care plans and person-centered approaches.
- Proficiency in using basic digital tools (e.g., email, electronic records, spreadsheets) to support case management and reporting.
- Relevant experience in health, social care, or community support settings; previous work with individuals with neurological or physical difficulties is highly beneficial.
- Formal training or qualification in health and social care, rehabilitation, occupational therapy assistance, or a related field is preferred.
- Ability to travel within the Manchester area and the wider region as required for client visits and team meetings.
