Associate Neuro Case Manager – Neuro Routes

We’re growing our associate network and are looking for experienced Neuro Case Managers to work with Neuro Routes on a self-employed associate basis.

We’re particularly interested in professionals with experience supporting people living with neurological conditions and/or serious or catastrophic injuries, who share our commitment to person-centred, collaborative and evidence-based rehabilitation.

The role

As an Associate Neuro Case Manager, you’ll manage a varied caseload, working collaboratively with individuals, families, legal professionals, insurers and multidisciplinary rehabilitation teams.

Your work may include:

  • Immediate Needs Assessments and rehabilitation planning
  • Coordinating multidisciplinary rehabilitation programmes
  • Setting and reviewing meaningful rehabilitation goals
  • Liaising with individuals, families and professional stakeholders
  • Monitoring rehabilitation progress and outcomes
  • Producing high-quality reports and case updates
  • Supporting independence, participation and quality of life
  • Advocating for the individual’s needs, wishes and goals

We’re looking for professionals who have:

  • A relevant health, rehabilitation or social care professional background
  • Appropriate professional registration
  • Experience working with neurological conditions and/or serious or catastrophic injury
  • Strong assessment, communication and report-writing skills
  • Experience of multidisciplinary rehabilitation
  • An understanding of safeguarding, consent, confidentiality and professional boundaries
  • The ability to work autonomously while contributing to a strong clinical governance framework

Experience within case management, personal injury or clinical negligence would be particularly welcomed.
