About the Role
We have an exciting opportunity for an Associate Practitioner to join our friendly and supportive team on Sapphire Ward at Gravesham Community Hospital.
Working within a rehabilitation setting, you'll play a key role in supporting patients to achieve their goals and maximize their independence. Working under the supervision of a Registered Nurse and as part of a multidisciplinary team, you'll deliver high-quality, person-centered care while developing and maintaining a broad range of clinical skills.

This is an excellent opportunity for an NMC-registered practitioner who enjoys working closely with patients and colleagues to make a meaningful difference to patient outcomes.
Main Responsibilities
As an Associate Practitioner, you will:
- Deliver safe, effective and compassionate patient care in line with organizational policies, procedures and professional standards
- Undertake delegated clinical interventions, including dressings, glucose monitoring and other agreed nursing procedures
- Support the assessment, planning, implementation and evaluation of patient care under the direction of a Registered Nurse
- Monitor patients' progress and promptly escalate any changes or concerns
- Maintain accurate and timely clinical documentation using electronic patient record systems, including EMIS
- Support rehabilitation programmes that promote independence and recovery
- Assist with ward administration duties, stock ordering and equipment checks
- Participate in multidisciplinary team meetings, audits and quality improvement initiatives
- Support the induction and development of new colleagues and students
- Maintain high standards of infection prevention, safeguarding, information governance and health and safety practices
- Contribute to creating a safe, caring and positive environment for patients, visitors and colleagues

Essential Requirements
- Current NMC registration
- Previous experience working within a health or social care environment
- Strong communication and interpersonal skills
- Ability to work independently and effectively as part of a multidisciplinary team
- Good organizational skills and the ability to manage competing priorities
- A compassionate, patient-focused approach to care
- Confidence using IT systems and willingness to learn new electronic patient record systems, including EMIS
- Ability to assess patient needs and deliver care within your scope of practice under the direction of a Registered Nurse
- Flexibility to work across a seven-day rota
- Commitment to ongoing personal and professional development
Desirable
- Previous experience within a rehabilitation, community hospital or intermediate care setting
- Competency or experience in:
- Venepuncture
- Catheter care
- Stoma care
- Wound management
- Experience contributing to audits, quality improvement initiatives or service development activities
