Job Introduction

As an Associate Practitioner within HCRG Care Group, you'll be part of a dedicated team delivering high-quality rehabilitation care to patients on Sapphire Ward at Gravesham Community Hospital.

About the Role

We have an exciting opportunity for an Associate Practitioner to join our friendly and supportive team on Sapphire Ward at Gravesham Community Hospital.

Working within a rehabilitation setting, you'll play a key role in supporting patients to achieve their goals and maximize their independence. Working under the supervision of a Registered Nurse and as part of a multidisciplinary team, you'll deliver high-quality, person-centered care while developing and maintaining a broad range of clinical skills.

This is an excellent opportunity for an NMC-registered practitioner who enjoys working closely with patients and colleagues to make a meaningful difference to patient outcomes.

Main Responsibilities

As an Associate Practitioner, you will:

  • Deliver safe, effective and compassionate patient care in line with organizational policies, procedures and professional standards
  • Undertake delegated clinical interventions, including dressings, glucose monitoring and other agreed nursing procedures
  • Support the assessment, planning, implementation and evaluation of patient care under the direction of a Registered Nurse
  • Monitor patients' progress and promptly escalate any changes or concerns
  • Maintain accurate and timely clinical documentation using electronic patient record systems, including EMIS
  • Support rehabilitation programmes that promote independence and recovery
  • Assist with ward administration duties, stock ordering and equipment checks
  • Participate in multidisciplinary team meetings, audits and quality improvement initiatives
  • Support the induction and development of new colleagues and students
  • Maintain high standards of infection prevention, safeguarding, information governance and health and safety practices
  • Contribute to creating a safe, caring and positive environment for patients, visitors and colleagues

Essential Requirements

  • Current NMC registration
  • Previous experience working within a health or social care environment
  • Strong communication and interpersonal skills
  • Ability to work independently and effectively as part of a multidisciplinary team
  • Good organizational skills and the ability to manage competing priorities
  • A compassionate, patient-focused approach to care
  • Confidence using IT systems and willingness to learn new electronic patient record systems, including EMIS
  • Ability to assess patient needs and deliver care within your scope of practice under the direction of a Registered Nurse
  • Flexibility to work across a seven-day rota
  • Commitment to ongoing personal and professional development

Desirable

  • Previous experience within a rehabilitation, community hospital or intermediate care setting
  • Competency or experience in:
    • Venepuncture
    • Catheter care
    • Stoma care
    • Wound management
  • Experience contributing to audits, quality improvement initiatives or service development activities
