We are looking for an enthusiastic, motivated clinician, who is willing to work flexibly within IDT, contributing skills to all parts of the team.
The primary purpose is to provide health and social care and carry out the assessment of patients in readiness for discharge from hospital, promptly and with the appropriate support from community teams. This is a varied and challenging role which encourages you to use a problem solving approach and to think outside of the box
This role is to work as an assistant practitioner in an identified cluster team within the integrated discharge team (IDT). This role works flexibly as an integral part of IDT, contributing skills to weekends and public holidays from 8am to 630pm

This innovative team has a ‘think home first’ approach to discharge and its primary purpose is to support ward teams with timely and safe patient discharge as well as assessing patients within the emergency department (ED) and assessment areas focusing on admission avoidance at the front door.
The IDT is based at Medway Maritime Hospital, Gillingham, Kent.
Main aims of the role – To work as a highly skilled practitioner without the direct supervision or presence of a professionally qualified member of the team.

- MCH encourages all staff to be double COVID vaccinated to protect themselves, their colleagues, their family and their patients.
