Clinical Support Worker

We are seeking an enthusiastic and motivated Clinical Support Worker to join our Frailty, Later Life and Dementia Service. The service supports older adults experiencing functional mental health difficulties, memory problems, dementia, and behaviours that challenge, while working closely with families and carers to provide person-centred care.
As a valued member of the team, you will work alongside qualified professionals to support, supervise and educate clients, helping them maintain their independence and wellbeing within their local community. You will work collaboratively with clients, carers, colleagues and partner agencies to assess needs, identify goals and deliver high-quality, person-centred support that promotes dignity, choice and positive outcomes.
This is a rewarding opportunity to contribute to improving the lives of older people while developing your skills within a supportive and dedicated multidisciplinary team.
The post holder will support the assessment, planning and delivery of person-centred care under the guidance of a supervising clinician, ensuring care is provided in line with clinical governance standards. They will undertake and accurately record physical health checks, including supporting individuals during clozapine titration and promoting their overall physical wellbeing.

Surrey is a beautiful county lying just 30 minutes away from Central London and from the South Coast. Our historic market towns and bustling districts are enveloped in wonderful countryside, and our excellent road and rail networks bring the rest of the country within easy reach. For international travel, both Gatwick and Heathrow airports are nearby.
