Later Life Neighbourhood Service Tandridge are seeking an enthusiastic community mental health nurse to join the team.
Role Overview
As a Community Psychiatric Nurse, you will work within a neighbourhood team, partnering with statutory, voluntary, and community sector organisations to deliver a coordinated, person-centred approach to care.
Main Responsibilities
- Provide specialist mental health assessment, intervention, treatment, and ongoing support to patients within their own homes and community settings.
- Act as a Care Coordinator/Lead Professional, where appropriate, ensuring high-quality care that promotes recovery, independence, and wellbeing.
- Undertake comprehensive mental health assessments and risk assessments.
- Deliver evidence-based nursing interventions and treatment plans.
- Work collaboratively with patients, carers, families, and partner agencies.
- Promote recovery-focused and person-centred care.
- Contribute to safeguarding, risk management, and clinical governance requirements.
- Support service development and quality improvement initiatives.
- Demonstrate good communications skills and able to work collaboratively within a multi-agency team.
