Role Overview

An exciting opportunity has arisen for Band 5 Nurses to join our team and work as part of urgent community response working in patient's homes across West Suffolk to deliver high quality nursing care.

Key Responsibilities

  • Assess, plan and deliver planned/unplanned healthcare for patients often with long term conditions and interventions will include: end of life care, palliative care, catheter care and wound care.
  • Enable patients to achieve quality of life, dignity and comfort.
  • Be passionate about delivering holistic care to patients and their families.
  • Work as part of an MDT who support urgent community response and at times will complete joint visits with ACPs and therapists. As a team we intercept some ambulance referrals to provide an urgent response.
  • Triage, assess, plan, implement and evaluate all aspects of patient care, mainly unplanned to include palliative and end of life care, catheter care and wound care.
  • Following holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required.
  • Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers.
  • Have a good understanding of palliative and end of life care, including symptom control and independent management of syringe pumps and end of life medication.
  • Use tactful and persuasive skills to gain the patient’s co-operation in their care management plan.
  • Liaise with GPs, Social Services, inpatient teams, hospice, other health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated.
  • Participate in multi-disciplinary/multi-agency meetings as appropriate.
  • To be competent to deliver IV therapy.
