Mole Valley Neighbourhood Mental Health Team

Role Overview

Mole Valley Neighbourhood mental health team is seeking an enthusiastic and experienced Senior Mental Health Practitioner to join our team. This role is for the Brief Interventions pathway and will work closely with GP Practices. This role is 33.75hrs and can be spread over 4 or 5 days a week.

Responsibilities

This rewarding role involves providing high-quality, person-centred care and support to adults with established mental health needs, helping them achieve recovery goals and maintain independence within their local community. The post holder will:

  • Manage a complex caseload
  • Undertake comprehensive assessments
  • Deliver psychosocial interventions
  • Work collaboratively with service users, carers, and partner agencies to ensure effective and coordinated care

Requirements

Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community-based work.

Key Responsibilities

  • Take a key role in developing and shaping a major new and transformative approach to providing mental health care within the community.
  • Work closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.
  • Work closely with local mental health services to support an easy in and out approach between secondary and primary care.
