RMN/RGN Role in Shared Care Protocol (SCP) Response Team

An exciting opportunity has arisen for an RMN to apply for the post of a Band 5 RMN/RGN in the new Shared Care Protocol (SCP) Response Team pilot across South & Central West Manchester Community Services.

Under The Line Management And Clinical Supervision From The Senior Nurse Practitioner (NMP), Consultant Psychiatrist And Relevant Senior Management, The Post Holder Will

  • Participate in the mental health nursing assessment, medication titration, monitoring and review of service users with an SMI who are prescribed antipsychotic and lithium medications.
  • Maintain and co-ordinate timely and appropriate interventions for an allocated caseload of service users of AOWA in the community operate in line with the relevant clinical pathways as outlined within the SCP Response Team Standard Operational Procedure (SOP).
  • This includes respecting individuality, values, culture and religious diversity of service users and colleagues and thus contributing to the provision of a high-quality service sensitive to these needs.

  • To utilise good interpersonal skills including specific skills to facilitate effective communication.
  • To establish and maintain good communication networks with service users, carers, and other health care workers and numerous external agencies.
  • To establish and maintain good working relationships with GPs, GP commissioner’s and relevant consortia.
  • To promote awareness of the role of SCP Response Team and work as link nurse to defined GP practices within local service area/team, and the wider community.
  • Be a full member of the multi-disciplinary team (MDT).
  • To participate in the assessment, monitoring and review of service users with an SMI who are prescribed antipsychotic and lithium medications.
  • To provide skilled mental health nursing care to service users.
  • To assess, develop, implement and evaluate programmes of care under supervision of senior team members.
  • To act as care case manager for service users under the SCP Response Team.
