This post forms part of our Band 5 rotations which offers experience in the core areas of; Orthopaedics, Trauma, Stroke (Acute and Community Based), Medicine, Surgery, Elderly Care, Community (including Intermediate Care), Paediatrics, Palliative, and our Integrated Discharge Team. Each rotation is 9 months (but may change according to service needs). Once fully developed into the Band 5 role and according to the needs of the service, the post holder may have the opportunity to develop more specialist skills.
The post holder will be responsible for assessing the needs of people on their caseload; devising appropriate treatment and rehabilitation plans, and identifying appropriate discharge pathways and services to enable people to return to their previous lifestyle where possible and practical.
As this is a Trustwide post, the successful candidate will be required to rotate between the Stockton or Hartlepool localities. We offer rotations in Orthopaedics (elective and trauma, Acute Medicine and surgery, Respiratory, Community Therapy, Stroke Rehabilitation (Acute and Community), Paediatrics, Integrated Discharge Team and Palliative Care.
