About the Role
The Band 6 Children’s Community Nursing Team Sister provides care in the home/community setting that would have been delivered previously in hospital. The post holder will carry their own caseload and provide supervision to other team members, when required. The team is further developing and now provides care to children in school requiring Total Patient as well as supporting existing service to meet the needs of children with chronic and other medical conditions at home thereby shortening or preventing hospital admission and improving school attendance.

The role is essential to provide nursing care within the community, being the lead professional for the CYP and linking in with the larger Multi-Disciplinary team including tertiary centres, social care, education and local consultants at the James Paget University Hospital.
Requirements
The post holder will need to have a valid driving licence.
