Role Description
As a Band 6 District Nursing Sister with the CSP qualification within the Evening and Overnight District Nursing Service for Central Manchester, you will provide high quality, autonomous nursing care to patients in their own homes. You will be responsible for assessing, planning, implementing, and evaluating nursing care, ensuring patients receive safe, effective, and compassionate treatment while promoting independence and preventing unnecessary hospital admissions.
Responsibilities
- Provide clinical leadership, supervision, and mentorship to junior staff and students.
- Work with GPs, OOH GPs, ambulance service, hospices, other community teams, as well as patients, family, and carers.
- Respond to urgent referrals and make informed clinical decisions in a variety of community settings.
- Undertake palliative and end of life care, catheter, and medication management while supporting patients with long-term conditions.
- Coordinate and prioritize the evening caseload to ensure timely and safe delivery of care.

Requirements
- Excellent communication, leadership, and organizational skills.
- Ability to work independently and prioritize workload.
- Ability to drive and have use of a car.
Hours
- 17:30 - 23:30, 18:00 - 00:00.
- May be asked to do a night shift with notice which is 22:30 - 08:00.
