Job Overview
An exciting opportunity has arisen for an experienced Band 6 Mental Health Nurse to join the Brent Older Adults Home Treatment Team. We are a small, friendly and supportive multidisciplinary team providing intensive crisis assessment, treatment and support to older adults in the community, helping to prevent hospital admissions and facilitate early discharge. Working in Brent, one of London's most diverse and multicultural boroughs, you will work closely with service users, carers and partner agencies to deliver high-quality, recovery-focused care, while supporting junior staff and contributing to service improvement.
Main duties of the job
- Provide specialist mental health assessment, risk assessment and crisis intervention for older adults experiencing acute mental health difficulties in the community.
- Deliver recovery-focused, person-centred care aimed at preventing hospital admission and supporting early discharge from inpatient services.
- Develop, implement and review individualised care plans in partnership with service users, carers and multidisciplinary colleagues.
- Administer, monitor and manage prescribed medication, providing advice and support to service users and carers.
- Support hospital avoidance and early discharge pathways by assessing referrals, providing timely crisis interventions, and contributing to decisions regarding the most appropriate care and treatment options.
- Work collaboratively with community mental health services, inpatient wards, primary care, social care and other partner agencies to ensure effective care coordination.
- Provide clinical supervision, support and guidance to junior staff, support workers and students.
- Maintain accurate clinical records and contribute to audit, quality improvement and service development activities.
- Participate in the team's duty system, shift rota and 7-day service provision.

Detailed Job Description And Main Responsibilities
The Brent Older Adults Home Treatment Team provides intensive, short-term assessment and treatment for older adults experiencing an acute mental health crisis as an alternative to hospital admission and to support timely discharge from inpatient services.
As a Band 6 Home Treatment Team Nurse, you will deliver high-quality, recovery-focused care to service users within their own homes and other community settings. You will undertake comprehensive mental health and risk assessments, develop and review care plans, provide therapeutic interventions, and support service users and carers through periods of crisis.
The post holder will:
- Undertake specialist mental health assessments, including risk assessments and mental state examinations.
- Deliver evidence-based interventions and crisis support to older adults experiencing acute mental health difficulties.
- Develop, implement and evaluate person-centred, recovery-focused care plans in partnership with service users and carers.
- Support hospital avoidance and facilitate early discharge from inpatient services through timely assessment and intervention.
- Manage a caseload of service users with complex mental health needs.
- Administer, monitor and review prescribed medication and provide advice to service users and carers.
- Work collaboratively with multidisciplinary colleagues, primary care, inpatient services, social care and voluntary sector partners to ensure coordinated care.
- Participate in MDT meetings, case reviews, discharge planning meetings and Mental Health Act assessments as required.
- Maintain accurate and timely electronic clinical records and contribute to quality improvement, audit and service development initiatives.
- Provide supervision, support and clinical leadership to junior staff, support workers and students.
- Participate in the team's shift rota and duty system to ensure effective delivery of the 7-day service.

Education And Qualifications
Essential criteria
- RMN/Diploma/Degree
- Evidence of continued professional development
Desirable criteria
- A post registration qualification in Older People
Experience
Essential criteria
- Post-registration experience in working with older adults or community working.
- Experience of working as member of a Multi –disciplinary team
Desirable criteria
- Experience of managing/supervision of staff
- Experience of supervising and assessing students
- Experience of working with older adults & dementia,
Knowledge
Essential criteria
- Knowledge of risk assessment and risk management processes within community mental health services.
- Knowledge and understanding of the needs and rights of service users and their carers.
- Knowledge of ethical/legal requirements within mental health nursing.
- Knowledge and understanding of legislation relating to the care of Older Adults with Mental Health problems in the community
