Are you an experienced registered mental health or social care professional looking for a rewarding role where you can help shape a developing service?
We are seeking a compassionate and motivated Specialist Practitioner to join the Incentivised Substance Free Living Team, part of the integrated Wellbeing Service at HMP Eastwood Park. Working within a multidisciplinary team, you will provide comprehensive mental health assessments and deliver evidence-based, recovery-focused interventions for people with complex mental health, wellbeing, and substance misuse needs.
- Manage risk, develop and review care and crisis plans
- Coordinate care
- Facilitate individual or group interventions
- Work closely with prison, probation, healthcare, voluntary, and community partners
- Provide clinical leadership, supervision, and support to colleagues and learners
Requirements:
- Hold a relevant health or social care qualification, such as RMN, Occupational Therapist, or Social Worker
- Current NMC or HCPC registration
- Substantial post-registration experience in adult mental health services
- Strong assessment and risk-management skills
- Knowledge of relevant legislation

This role focuses on providing appropriate, effective interventions and treatments to people with a range of mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care, and well-being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
- Work autonomously to undertake emergency assessment in a wide range of locations
- Planning and delivering defined, intensive, specialist interventions in line with the CPA framework to people with severe and enduring mental health needs, who may be distressed and in crisis
- Work alongside other services to plan, deliver, and review effective “wrap around” intensive interventions to manage risk and prevent hospital admission for those with the most complex needs
