Job Overview
Leicestershire Partnership NHS Trust are working together with Adult Social Care across Leicester, Leicestershire and Rutland (LLR) in order to deliver our HomeFirst service offer alongside core Community Therapy.
We have had significant investment in staffing in order to ensure that services can be delivered by the right therapist in the right place at the right time, and to ensure that the local population across LLR receive timely high-quality therapy. This is therefore a really exciting time to be joining the service to: support people to remain in their homes when they are having a health or social care crisis, rather than needing to go into hospital or a care home
We are really proud of this recent feedback for our new service offer: “I really didn’t want him to go back to hospital, he doesn’t get on well there, he doesn’t know anyone and they don’t really know him. He’s only just come out and he got another infection when he was there. We have got one carer once a day and they couldn’t get him to stand up on Thursday night. By Friday he had been stuck in his chair all night as he was too tired and weak to get up, the Home First Therapists came in and helped. They were so kind; they got him out of the chair and into bed. They made him feel comfortable; they treated him like he was special and not just a number, they made me feel better to”
Do you have the ability to deliver similar high quality compassionate care and think you will be the right person to join our team which is already proven to make a difference? We are looking for Band 6 Physiotherapist to provide an effective and efficient therapy service.
This post requires a level of personal responsibility and we are looking for someone highly motivated, resilient and proactive with the ability to prioritise and manage their own allocated caseload and work, use their initiative and the ability to work on their own and as part of a team. You will demonstrate excellent communication and organisational skills and be able to adapt to a changing and pressurised environment.
You will work alongside Occupational Therapists and Technical Instructors carrying out comprehensive functional assessments and interventions in order to help our patients live as independently as possible. The therapy teams operate in an integrated way whereby Occupational Therapists and Physiotherapists are encouraged to work synergistically in order to provide the best possible outcomes for our patients. You will be encouraged and supported to grow your skillset, and to challenge the way we do things. We are looking for determined and self-motivated individuals who can make a difference to the care of patients from all backgrounds, and to promote therapy services within the trust.

There may be a number of posts available across the City and County. As a service we operate Monday to Friday 8-6 and Saturday/Sunday 8-4 and 8-4 on Bank holidays. The candidate will be expected to work a range of shifts across the week.

Job Description Summary (Main Duties And Responsibilities)
- To work as a specialist clinician to utilise, develop and enhance Physiotherapy clinical and professional skills within the Service to deliver high standards of practice.
- To provide specialist Physiotherapy assessment and treatment to patients within a multi-disciplinary format in clinical care pathways.
- To take a lead in developing the Service. This involves promoting high quality clinical practice and ensuring professional standards are maintained in the workplace.
- To deliver a specialist Physiotherapy Service to patients within a variety of settings, requiring flexibility/adaptability in recognising the complexity of these environments to maximise their abilities and opportunities for occupational wellbeing.
- To be responsible for own practice and accountable for all aspects of the Physiotherapy assessment and treatment (information gathering, assessment, treatment planning, intervention, evaluation and discharge planning) as appropriate working with a person centred approach.
- To assist team/clinical/operational leaders in the organisation and co-ordination of the service provided. To manage own time constructively and prioritise clinical workload working with patients on a 1:1 basis or in a group.
- To work in partnership with team leads, managers and other practitioners in the effective management of referrals, case prioritisation and care co-ordination within clinical care pathways.
- To work effectively with members of the Multi-Disciplinary Team and other related agencies to ensure a needs-led service is maintained.
- To attend and/or lead clinical case reviews and present appropriate Physiotherapy feedback accordingly. To work flexibly and in partnership with the multidisciplinary team, to provide a holistic approach in meeting the needs of patients and the service.
- To work within an advisory capacity as appropriate within the post and skills/experience of post holder in order to inform others (services, family, carers, agencies) regarding patient needs.
- To delegate workload effectively to other team members. To communicate and work collaboratively with patients, families/carers, other disciplines and other agencies to ensure good professional working relationships are maintained.
- To have clinical responsibility for a designated caseload of patients and to organise this effectively with regard to clinical priorities and in line with the Trusts waiting/response times and service priorities.
- To communicate effectively and work collaboratively with patients and families/carers to maximise an individual’s wellbeing. Communication skills of explanation, motivation, negotiation and sensitivity will be used in a variety of situations.
- To participate in all relevant clinical team and professional meetings to contribute to the optimum level of care for patients and ensure effective liaison with other agencies, families/carers and to contribute to effective team working and development.
- To deliver supervision to Band 5 or unregistered staff as required. To support and deliver training in relation to patient needs.
- To participate in and lead where appropriate in-service training within Physiotherapy and the wider service. To demonstrate the Trust’s values in everything you do in the work environment and live up the LPT Pledge.
- To be responsible in the use and expenditure of the Trust’s resources that you utilise.
